Don't think you need a specific template. There isn't one published by hse so it should be a fairly simple generic type.

Should be fairly straightforward.
Knock / ring doorbell and stand back, same as couriers / postmen are doing, or better still phone from van to warn of arrival.
Make sure customer knows to stay 2 metres away, preferable not in same room work being done.
Tea / coffee / cake / biscuits / sandwiches to be left in agreed place.

Arrange visit by email confirming you and your family are well, ask customer to do the same

arrive at property, using hand cleanser before entering, ensure social distancing

decline tea , coffee, biscuits

after leaving , use hand cleaner again

repeat

not sure what else you can do ....... other than not do the work

I'd add 'ventillation' to your list. Work with windows open. Ask client to open them before you arrive and give area a chance to blow through. Advise them to leave them open for some reasonable time, say 30 minutes, after you leave.

Emphasis on droplets and aerosols as a risk factor seems to have been climbing over the last couple of months.
