I detected a gas escape when carrying out a gas tightness test of the NG
meter and gas installation pipework up to and including 28 mm diameter with
appliances connected for gas tightness in accordance with the Institution of
Gas Engineers and Managers Standard IGEM/UP/1B Edition 3.Tightness
testing and direct purging of small Liquefied Petroleum Gas/Air, Natural Gas
and Liquefied Petroleum Gas installations. The gas escape rate was not within
the accepted tolerances of IGEM/UP/1B Edition 3.

gas supply in an emergency situation. British Standard 6173:2009
Specification for installation and maintenance of gas-fired catering appliances
for use in all types of catering establishments (2nd and 3rd family gases)
requires that a manually-operable valve shall be fitted in the gas supply to
every catering area to enable isolation of the supply in an emergency. Either
this valve shall be located outside the catering area or near an exit, and be in a
readily accessible position, or an automatic electric valve system shall be fitted.
In the latter case, an emergency stop button or control shall be located to
enable operation in an emergency. The control shall either be shielded or

We recently visited your property to do an inspection on the gas work carried out by Unknown .. You can read the findings
from this inspection in the Unregistered Investigation Report overleaf.
Unregistered gas work
The work recently undertaken at your property has not been carried out by a Gas Safe registered business. By law, only
Gas Safe registered engineers can carry out gas work. The Gas Safety (Installation and Use) Regulations 1998 make
it a legal requirement that only suitably competent Gas Safe registered engineers are permitted to install, commission,
service or repair gas pipe work or appliances (e.g. boilers, water heaters, fires, cookers, hobs etc). A person convicted of
carrying out gas work when not registered with Gas Safe Register could face:
• a fine of up to £20,000 and up to six months imprisonment in a Magistrates’ Court (or Scottish equivalent), or;
• an unlimited fine and up to two years imprisonment on indictment in the Crown Court.
For multiple breaches, the penalties may be significantly higher.

As the person responsible for your property, you should not use a gas appliance or allow someone else to use it if you
know or suspect that it could be dangerous. This is a requirement of Regulation 34 of The Gas Safety (Installation and
Use) Regulations. If checks and tests have identified gas safety defects, they are in the enclosed Defect Report section
of the Unregistered Investigation Report. Where gas safety checks and tests have not found unsafe defects, with your
permission the gas installation or appliance may have been left working. You will have received an Unregistered Gas
Work Advice Notice form. In the interests of safety, as part of our inspection we may have issued you with a Warning
Advice Notice. You could be putting your family, friends or other occupants of the property at risk if you use an appliance
or gas fitting classified as unsafe. It could cause a gas leak, fire, explosion or carbon monoxide poisoning.

On the 31 March 2014 I went XXXXXXXX as a complaint had been
received by Gas Safe Register. The complaint has been allocated a reference number of XXXX.
At the above address I met XXXXXX(EHO's, East Staffordshire Borough Council) and XXXX
(Staffordshire Fire and Rescue), also XXXX Heating and Plumbing a Gas Safe Registered engineer
who had worked at the property in December 2013. A gas escape had been identified at the property and the gas supply
disconnected by the gas emergency service provider. *** the cause of the gas escape and
repaired it and restablished the gas supply.XXXXX does not have the relevant ACS competencies to work on
commercial catering installations.
During the inspection as a result of a significant gas escape being identified soon after arrival at the property a visual only
inspection was then carried out.
While carrying out the inspection numerous issues were identified and are listed within the defect section of the report, as a
result of these defects the installation was classed as Immediately Dangerous in accordance with the Gas Industry Unsafe
Situations Procedure (GIUSP).
The commercial kitchen areas located at the property had mechanical extract systems which required natural ventilation.
The extract canopies were not fully checked during the inspection; therefore the responsible person(s) for the property are
advised to have the extract systems, and make up air provision checked/inspected by a suitably qualified person due to the
following reasons;
• Excessive ambient air temperature in the front catering/restaurant area,
• Appliances not suitably positioned under the canopy in the rear catering area
• No visible make-up air provision
• No evidence was available on site to confirm safe levels of CO² within the catering area.

Pretty much what I find at very one of these establishments, worst one was no interlock or vents or make up air, I swear you could not breath
