Aesthetic reasons, however you are right it's not worth the botherThen why do it ? When you can have a surface mounted one the pipework can be concealed
Personally I think the surface mounted showers can be better looking and not much more bulky than the "built in" type. The outlet for hose is part of shower, so no silly plumbing for that also. And easier to service or replace in the future.
